Hip Fracture and Fragility Fracture Prevention Project

 

Fracture-33The principle aim of this project is to improve the health outcomes for patients at risk of fragility fracture and promote improvements in bone health through targeted case finding, effective treatment and medication compliance.

Through active case finding the project aims to both proactively manage high-risk patients for primary prevention of fragility fractures whilst also ensuring optimal navigation of treatment pathways for secondary prevention strategies.
