Better COPD Care – A COPD Coordinator Role as a Catalyst for Multi-CCG Innovation, Dissemination Wealth Creation and Guideline compliant care

Chronic Obstructive Pulmonary Disease (COPD) is a major cause of preventable hospital admissions across the North East and North Cumbria region and there are significant variations within the regions in access to best COPD care. This project will roll out interventions in Newcastle-Gateshead CCG alliance footprint that are recognised as cost effective in NICE 2010 COPD guidelines.

Self-management plans are cost effective yet poorly implemented. Wider use offers the potential to reduce primary care consultations, reduce hospital admissions and free up resource to treat other patients.

Deliverables on this project include education sessions, a baseline audit of COPD activity across primary and secondary care, introduction of a standardised COPD chronic management pro-forma, introduction of a standardised exacerbation plan, attracting and start of an industry trial in COPD, a register of 300 new COPD patients and a dissemination event to roll out the work to other areas in the North East and North Cumbria.
